Data Subject Privacy Requests
The Acrisure Privacy Request portal is currently undergoing maintenance, and we are unable to presently take in requests through this method. Please rest assured that your privacy continues to be our top priority and you are still able to submit any requests to exercise your privacy rights by calling us at 877-504-9487 or by sending us an email at [email protected]. When submitting a request via email, please include the following information to facilitate the processing of your request:
• Your full name
• Your address
• The type(s) of request(s) you are submitting (e.g. Request to: delete personal information, access personal information, correct personal information, etc.)
If you are looking to exercise your rights pursuant to the Privacy Notice you received from Acrisure (either via mail or through the Acrisure Customer Portal), which would include your right to opt-out of certain sharing of your financial personal information, please include the Customer ID that can be located on the Privacy Notice sent to you. If you reside in California, you can also exercise your rights under CalFIPA by printing and completing the form located here, which can then be mailed to: Acrisure, LLC, 100 Ottawa Ave SW, Grand Rapids, MI 49503.
We will reach out to you once your request has been processed but may need to reach out before that if additional information is required to process your request.
